Important Information for your first visit:

Patient's with HMO insurance plans may require an insurance referral from their primary care physician. This may be required even if you have an order or referral from a specialist or surgeon. This referral should be requested from your primary care physician prior to your first appointment. The primary care will need the National Provider Number  (NPI) for this office which is 1386786770. The referral should be made for occupational therapy. Please note, referrals for physical therapy will not be processed properly and may not be covered.  If more than 1 visit is expected, please request the anticipated number of additional visits to avoid multiple referral requests for subsequent visits.  Please contact this office with any questions re: HMO plans.

• Please bring a prescription from your referring physician.

• If your insurance requires a referral from your primary care physician, then please contact his/her office prior to your first visit with our national provider identifier (NPI) to properly process the referral. NPI #: 1386786770  Referral authorization can be faxed to (508) 748-3944.

• Remember to wear loose fitting clothing and/or a tank top if you are being treated for a shoulder injury.
